What is the mission of your practice?

Our goal is to provide the highest-quality endocrine care and service, emphasizing a proactive, comprehensive approach to both disease prevention and wellness. From the moment you enter our office, we want you to be completely satisfied with every aspect of your care.

How can I schedule an appointment?

Kindly call our clinic on 561-889-0388 and our staff will be more than happy to help you book an appointment

What are your office hours?

Except for holidays, our office hours are Monday through Friday from 9:00 am to 5 pm.

How do I refill my prescription?

Please Call your pharmacy and request prescription refill. Kindly allow 3 business days for it to be filled. Medications ordered by your primary care physician or other specialists need to be refilled by them.

What do I bring with me when I return for follow up visits?

We recommend you bring your updated list of medications and current insurance cards. Your follow up appointment is also the best time to bring re-fill requests for your endocrine medications as well as the address and fax number of the pharmacy you go to. If you have diabetes, please bring your blood sugar log with you.

Why are follow up visits important?

Endocrinology is a specialty that treats chronic medical conditions. The chronic nature of these disorders necessitates periodic clinical reassessment to ensure that treatment goals are being attained and optimal care provided. This also helps us monitor our patients for potential side-effects of therapy and any complications arising from the underlying disease state. The evolving nature of medicine is personalized care, and rather than having a ‘one-size-fits-all’ approach to your treatment, your follow up visit allows us to create a care plan based on your individual needs.

Do you accept Self pay if I do not have insurance or if the clinic is out of network?

Yes, we do not want your insurance plan or financial situation to be in the way of receiving medical care at our clinic. Please call the office on 561-889-0388 to learn about are discounted self-pay fees.

Do I have a co-payment?

Please call your insurance carrier for plan benefits. However, it is our experience that most plans have patient responsibility portions described as deductible, co-insurance, and co-payment amounts. We understand this can be a complex matter and as a courtesy, our front desk staff will be more than happy to confirm and clarify your benefits with the insurance company.

Who can help me with billing questions?

Billing is done by a third-party billing company. You can reach the billing department by calling 561-889-0388 and selecting option 5. They will gladly answer any question you may have regarding your statement. Patient co-payments, co-insurances and deductibles are paid at the time of visit. Should you have any questions regarding your co-insurance and deductible amounts, our office can gladly supply you with estimates. Of course, the insurance carrier is always the best source to call with any questions regarding your plan benefits.

Can I call after hours for medical emergencies?

If you have a medical emergency, please call 911 immediately. Otherwise, you may call during regular office hours. Prescription refill requests are not considered medical emergencies and must be called in during regular office hours. If you have a very urgent Endocrinologic matter to discuss with us, please call us and the doctor will contact you asap.

Can I review my diagnostic and laboratory results by phone?

Optimal care requires face-to-face interaction between a patient and health care provider. If a result is of high priority, the doctor will call you as soon as he reviews the result. There will be no delay in addressing urgent or abnormal lab results. Our patient’s health is our priority.

What is your clinic policy for cancellation and No show?

Please remember to call and cancel your appointment. Failure to do so prevents other patients from being seen. Our policy requires 24 hours’ notice. Our office will charge $50.00 administrative fee if you fail to show up to your scheduled appointment or fail to provide 24 hours’ notice when cancelling an appointment. Two consecutive “NO SHOWS” may jeopardize your future appointment availability and is subject to a discharge from our practice.

How Can I request my medical records?

The form can be found in our Patient forms tab on our website. Please complete, sign, and mail or fax to 561-8551655. The completed form can also be emailed to medicalrecords@endocrineboca.com. We will fax the records for free. Printed hard copy records service fee is $20.00. Please give five business days for the request to be completed.
