Please Call our practice on 561-889-0388, Monday-Friday from 9:00am to 5:00pm to schedule an appointment. You can also submit an appointment request by clicking below and we will be in touch with you asap.
Check in online before your visit
· Few days before your visit, you will receive an appointment reminder. Please confirm the appointment. You will also be able to conveniently sign the check in forms and complete the medical questionnaire before your appointment using your phone or computer. This will save you time on the day of the appointment and this will give us a chance to learn about you even before your visit.
· Please ask your referring physician’s office to fax your medical records at least one week prior to your visit, to 561-8551655
Bring the following to clinic
• Driver’s License/ Photo ID
• Insurance Card/s
• Referral from your PCP if required by your health plan
• Any relevant lab report/ diagnostic test report
• Updated List of medication with their dosage
• If diabetic, please bring your blood glucose log
• Form of payment: Credit Card/ Cash/ Check.
